How to Enroll
For Members enrolled in one of the two Aetna plans offered through the Institute:
- Complete a Subsidy Application and submit to the Office of Human Resources at Fuld 102.
For Members enrolled in private insurance:

Complete a Subsidy Application and submit to the Office of Human Resources at Fuld 102 along with proof of insurance specifying the following:
- Length of Contract (period of coverage)
- Type of Coverage (single, family, employee +1)
- Total Cost of the premium in US Dollars
- Proof of payment (i.e. Xerox copy of receipt)
NOTE: Subsidy does not cover AD&D, Dental or Vision.
We can only process the subsidy after receiving the documentation listed above.
A new application and proof of insurance must be submitted for each academic year.
Reporting Changes
Your eligibility will remain in effect unless your status changes from Member to Visitor or you have a change in family status that affects your current coverage/premium. Please notify HR of any changes at the time they become effective.
